FIELD: medicine.
SUBSTANCE: during first stage under local anesthesia ultrasound-guided percutaneous transhepatic cholangiostomy is conducted simultaneously with microcholecystomy. After that microcholecystostomic drain is replaced with a guide wire. Through the guide wire gradual telescopic bougienage of cholecystomy channel is performed reaching the diameter fitting the diameter of rigid optic instrument. Contact lytholapaxy from the area of cystic-choledocheal fistula, gallbladder, hepaticholedocha is performed tthrough the operating channel of the instrument. Finishing the manipulation insuring drain is applied into the lumen of the gallbladder. If there is adequate patency of biliary tree, drainages are temporally blocked for 24 hours. Cholecystomy and cholangiostomy drains are removed.
EFFECT: being conservative surgery, it is easily tolerated by patients, and ensures fast reduction of intoxication and pain relief.
