FIELD: medicine; orthopaedics and traumatology.

SUBSTANCE: osteoinductive coated pins are delivered through greater trochanter, neck defect zone, and femoral head fixed in a transosseous apparatus. Transverse osteotomy is performed through the base of greater trochanter at the level related to the inferior head pole. A distal femoral fragment is medialised firmly to the head and led aside to ensure its external surface contacting with osteotomised surface of a proximal fragment. The second transverse osteotomy is executed in distal direction from a contact point. The bone fragment formed thereby at the level of femoral neck is fixed with penetrating pins tensioned for compression efforts within neck defect zone. By reducing the limb, neck-shaft angle is restored. The distal fragment is displaced ectad and upwards to contact with osteotomised surface of greater trochanter. Femoral fragments are fixed with a transosseous apparatus wth generating compression efforts. The operation is followed with additional osteotomy through femoral shaft with distal femur moved longitudinally.

EFFECT: restored correct anatomical arrangements in proximal femur, restored anatomical length of limb, consolidation within defect zone, reliable fixation, reduced treatment time.
