FIELD: medicine; surgery.
SUBSTANCE: expander dermotension is produced. It is ensured by making two incisions of length fitting with diametre of a trocar from intended boundary of the expander at the distance not less than length of its valve tube. The incisions are spaced 5 cm apart. A trocar is inserted in each of them: an endoscope is inserted into one incision to perform control, while endoscopic dissector is inserted into the other. The expander bed is formed by pneumatic preparation of a tissue-like layer that is combined with electrocoagulative haemostasis. Then the endoscopic dissector is removed, while the incision is widened to the length that fits the diametre of the tube-type conduplicate expander. With using an endoscopic manipulator, the conduplicate expander is inserted into the formed bed. Then the expander is unfolded and filled with physiologic saline to complete straightening. Then the expander valve is fixed; the endoscope, endoscopic manipulator and trocars are removed; skin wounds are taken in tightly.
EFFECT: plasty of extensive skin defects, improved cosmetic result, reduced treatment time.
