METHOD OF PLASTIC CLOSING OF DEFECT IN THE SURGICAL TREATMENT OF SKIN MELANOMA Russian patent published in 2004 - IPC

FIELD: oncology.

SUBSTANCE: invention is designed for use in surgical treatment of skin melanoma complicated by plastic inflammation of postoperative defect with local tissues in case of large excision of tumor. Method comprises limitation of proliferation of melanoma by bilateral skin cuts taking into accounts lymph flow routes, creation of expander bed, introducing expanders, intraoperatively filling hollows of expanders with cold physiologic solution. Since the first day of the postoperative period, expanders are forcedly filled with physiologic solution through catheters to induce extension of skin above them. 5 hours later, physiologic solution is partly evacuated in order to restore blood circulation. Over 10-day period, forced filling and partial evacuation of physiologic solution are performed each time followed by short-focus X-ray therapy in summary focal dose 50 Grey onto the primary focus. Later on, expanders are removed through skin cuts available and, they being extended, primary melanoma focus is widely excised and postoperative defect is plastically closed of excess of skin obtained as the result of dermotension.

EFFECT: enabled creating, over a short period of time, additional skin reserve, increased efficacy of surgical intervention, reduced risk of postoperative complications, and improved rehabilitation of patients.
