METHOD FOR PLASTIC COVERING THE DEFECT AT SURGICAL TREATMENT OF CUTANEOUS MELANOMA Russian patent published in 2005 - IPC

FIELD: medicine, oncology.

SUBSTANCE: it is necessary to develop a channel for an expander at lateral body surface in area of iliac wing. One should introduce expander into subcutaneous-fatty cavity, through contraperture it is necessary to withdraw expander's catheter onto skin surface to suture skin incision. Intraoperationally one should fill expander's cavity with cold physiological solution of sodium chloride, then, since the first d of postoperational period one should perform forced filling the expander with physiological solution of sodium chloride through catheter till skin stretching above it. Moreover, 5 h later, one should partially evacuate physiological solution to normalize circulation. For 10 d, daily, one should repeat forced filling of expander and partial evacuation of physiological solution. Simultaneously, one should perform the course of distance gamma-therapy at dose fractioning per 3 Gy, total location dose 30 Gy onto area of primary focus and area of regional metastasizing. Then one should remove the expander through skin incision to prolong it, with bordering skin incision one should form free skin transplant. One should widely dissect primary focus and perform plastic covering the postoperational defect with free skin fragment. The method enables to develop additional skin reserve in the shortest period of time.

EFFECT: higher efficiency of plastic defect covering.
