FIELD: medicine.
SUBSTANCE: invention relates to field of medicine, namely to vascular surgery and angiologia. Method of half closed iliac arteries endarterectomy includes dissection of general femoral artery, peeling off plague by spatula from vessel wall, introducing Volmar's ring stripper into general femoral artery lumen, with work part in form of ring with cutting edge, located on internal side of loop, in place of handle fastening. During Volmar's ring stripper introduction placing of proximal plague end inside of ring is carried out. Moving of ring is carried out in proximal direction with sequential passing of areas of occlusion or stenosis of general femoral and external iliac artery till sensation of "gap", in direction opposite to blood flow movement. After passing each area removal of instrument with plague is carried out. Restoration of central blood flow is checked and in case of its absence or considerable reduction ultrasound ultrasonic detector is placed in projection of general iliac artery and duplex scanning is carried out and under its control ring stripper is introduced into general femoral artery lumen, with work part in form of ring with cutting edge, placed on internal side opposite the place of handle fastening. Said ring stripper is passed parallel to stenosing plague to orifice of general iliac artery and further into terminal part of abdominal aorta. Ring is turned towards aorta bifurcation end of plague, is caught by ring. Deobliteration of general iliac artery in blood flow direction is carried out. If necessary manipulation is repeated, applying ring stripper with ring of larger diametre.
EFFECT: increasing efficiency of half closed endarterectomy in case of diffused affection of external and general iliac arteries, from small, low traumatic transfemoral access, due to application of modified instrument while performing second stage of operation under control of intraoperation duplex scanning.
