FIELD: medicine, vascular surgery.
SUBSTANCE: one should fulfill retroperitoneal mini-access towards terminal aorta and iliac arteries due to dissecting the skin, subcutaneous fiber, aponeurotic fascia of external oblique muscle of abdomen, then it is necessary to change the line of incision: in proximal and distal directions strictly vertically along external edge of rectus muscle of abdomen sheath one should dissect Spiegel's line, then along Kan's line it is important to fulfill the access towards femoral arteries. One should fulfill longitudinal arteriotomies above the aorta at transition onto left iliac artery, above internal iliac arteries, above the mouths of deep femoral arteries. Detachment of atherosclerotic plaque should be fulfilled with the help of ultrasound blade, the plaque should be crossed crosswise. The lumen of aorta, internal iliac and deep femoral artery should be reconstructed similarly due to open technique. Ultrasound disobliteration of iliac arteries should be carried out with metallic rings, moreover, one should start to apply the ring from femoral access in proximal direction. Removal of atherosclerotic plaque should be fulfilled out of retroperitoneal mini-access. The innovation declared enables to decrease surgical trauma, achieve decreased pain syndrome in early post-surgical period, carry out earlier activation and rehabilitation in such patients, decrease the quantity of post-surgical complications, such as intestinal paresis and pneumonia.
EFFECT: higher efficiency of therapy.
