MASCULINISING URETHROPLASTY TECHNIQUE IN TRANSSEXUALISM TREATMENT Russian patent published in 2009 - IPC A61B17/00

SUBSTANCE: invention concerns medicine, plastic surgery, can be used in consecutive sexuality operations. Perineum is reconstructed by excising vaginal mucous membrane from urethra herewith forming two independent portions. Mucocutaneous part is rolled up inside to form two isolated tubes - vaginal and urethral, with outlet openings located in lower and upper parts of perineum. Formed vaginal tube is delivered into area of lower pole of the wound by lesser fistula. A Foley's catheter is inserted into bladder through the urethral tube. After hydropreparation of perineum tissues with dissolved anaesthetic and vasoconstrictive preparation, vaginal mucous membrane is isolated from urethra by dissection of mushrooming herewith forming the independent departments - vaginal and urethral. Anterior wall of vagina is separated from posterior wall of urethra along 2-2.5 cm at depth 0.7-1 cm. Interposition of separated tissues of anterior wall of vagina follows between anterior wall of vagina and posterior wall of urethra. Perineum urethra of length 1.5-2 cm is formed.

EFFECT: method allows improving functional and aesthetic effects, reducing surgical risk and surgical injuries, improving clinical effectiveness owing to elimination of damage of a donor hand and a back, reducing mobilisation of local tissues and shortening of perineum neourethra; reducing risk of external neourethral urinary fistulas, providing adequate male urination function, allows for reverse vagina restoration.
