METHOD FOR IDENTIFICATION OF OPTIMAL MODES OF FLUORESCENT DIAGNOSTICS AND PHOTODYNAMIC THERAPY Russian patent published in 2009 - IPC A61N5/06

FIELD: medicine.

SUBSTANCE: invention is related to medicine, namely to oncology and may be used for execution of clinical tests of new photosensitisers. Optimal interval of time is defined between introduction of preparation and session of fluorescent diagnostics (FD) and/or photodynamic therapy (PDT). Optimal interval of time is identified on the basis of two parametres - maximum fluorescence of photosensitiser in tumor and maximum fluorescent contras of photosensitiser content in tumor relative to surrounding unchanged tissues. Terms of light mode observance are defined, for this purpose kinetics of preparation distribution in skin is defined, as well as time for excretion of photosensitiser from patient body by its pharmacokinetics in blood and urine. Optimal dose of preparation for FD is defined, and/or optimal doses of preparation and laser radiation for PDT, besides comparison of various doses of preparation and doses of laser radiation is done in process of FD and/pr PDT sessions in optimal interval of time after introduction of preparation, and for comparison doses are selected from range of doses proposed as a result of preclinical tests. Optimal dose of photosensitiser for FD is accepted as dose of preparation, with which the following conditions are achieved: bright fluorescence of preparation in tumor, maximum fluorescent contrast between tumor and norm, minimum fluorescence in foci of inflammation and foci of nonneoplastic pathology, besides if these indices do not differ for various doses, then optimal will be the dose, in case of which the lowest skin phototoxicity is obtained. Optimal therapeutical dose is accepted as the dose, in case of which there is no reduction of therapeutical efficiency with application of identical doses of laser radiation in combination with low skin phototoxicity. Optimal laser radiation dose is identified, for this purpose several doses of laser radiation are used in session of treatment in optimal time after introduction of preparation in optimal dose, and optimal dose of laser radiation is accepted as dose, in case of which the highest clinical efficiency is achieved, being assessed by number of complete regressions of tumor at lowest number of complications.

EFFECT: method makes it possible to reduce terms of optimal FD and PDT modes development, and also to reduce number of complications during FD and PDT due to elimination of application of high doses of preparation and laser radiation.
