FIELD: medicine.
SUBSTANCE: invention refers to the field of medicine, namely to purulent surgery. The method consists of revision of pancreas and retroperitoneal fat, removing necrotic masses, multiple drainage of saclesser omental sac cavities and retroperitoneal space, daily dynamic medical sanitation of saclesser omental sac, pancreas and retroperitoneal fat implemented conducted using ultrasound impact on the treated solution. At that this drainage of saclesser omental sac cavities and/or retroperitoneal is performed by means of drainage device equipped with a conductor of NiTi with shape memory, an additional window in its central part and two branches of perforated working ends. Medical sanitation of saclesser omental sac, pancreas and retroperitoneal fat, as well as ultrasound impact on washing solution is performed through an additional hole in the drainage device. During drainage of saclesser omental sac cavity branches of perforated working ends of the drainage device are placed respectively in the left and/or right subhepatic and subphrenic spaces, and the additional hole is placed just above the pancreas, and while draining retroperitoneal space branches of perforated working ends of the drainage unit are placed respectively from the left and/or right lumbar regions, and an additional hole is placed over the saclesser omental sac from the pancreas posterior surface and over the retroperitoneal fat. The proximal end of the drainage device is put out through puncture of the skin in the most declivious site of the left and/or right side of lateral canal. The insertion of the drainage system into the retroperitoneal space is conducted by means of the device for rethroperitoneoscopy, at that prior to the drainage systems insertion into the retroperitoneal space the ultrasound transducer is inserted through the same device into the retroperitoneal space and the ultrasound impact is made in antiseptic solution on the pancreas inflamed tissue from its rear surface and the retroperitoneal parapancreatic fat within 5-7 minutes of AFC 40±2 kHz. Ultrasound impact on the treated solution at the daily medical sanitation of saclesser omental sac, pancreas and retroperitoneal fat is carried in the mode of AFC 70 ±20 kHz for 5-7 minutes.
EFFECT: application of this invention enables to improve the treatment efficiency in patients of this category.
