FIELD: medicine.
SUBSTANCE: invention relates to field of medicine, namely to vertebrology. In course of selection of tactics of treating patients with degenerative affection of spine performed are radiologic, computer-tomographic (CT) and electroneuromyographic (ENMG) examinations. Attention is focused on indices of: height between vertebra bodies at the level of affected vertebral-motor segment, narrowing of spinal canal, deformation of dural sac and roots, state of spinal marrow motoneurons activity. After that spine traction under ENMG control is performed. Value of spine traction force is selected and left at the level at which constancy of values of spine marrow motoneurons activity is achieved. CT control is carried out at earlier selected value of traction force of: dynamics of change of spine canal lumen, deformation of dural sac and roots at the level of affected vertebral-motor segment, restoration of height between vertebrae bodies at the level of affected vertebral-motor segment. Obtained values are compared with values of initial indices. Degree of their changes is evaluated. Depending on them tactics of treatment is selected. In case of unchanging state and constancy of index values surgical treatment with spinal canal revision is prescribed. In case when observed is essential positive dynamics in change of index values in deformation of dural sac and roots, restoration of height between vertebrae bodies at the level of affected vertebral-motor segment and depending on decrease of narrowing of spinal canal lumen to a half of initial value, interspinal dynamic fixation of affected spine segment is prescribed. In case when narrowing of spinal canal lumen reduces per a half and more of initial value, puncture decompression of affected disc by hydroplasty method is prescribed. In case of virtually complete restoration of indices in process of said examination and control, puncture decompression of affected disc by electromagnetic exposure in traction mode is prescribed.
EFFECT: method ensures realisation of rational selection of adequate treatment taking into account state of neuromuscular apparatus and character of changes under impact of traction of spinal canal lumen.
