METHOD FOR VIDEO-ASSISTED ENDOLARYNGEAL RESECTION WITH THERMAL ABLATION IN LARYNX CANCER TREATMENT Russian patent published in 2010 - IPC A61B5/00 

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to oncology, and is intended for larynx cancer treatment. The method involves inferior tracheostomy, introduction of instruments into the laryngeal cavity and tumour removal. Herewith, tracheostomy is followed with direct rigid laryngoscopy through the mouth. After larynx structure visualisation, a laryngoscope is fixed on the thorax with optical fibres fixing to the laryngoscope. Further, high-frequency thermal ablation of the laryngeal tissues is enabled by a related apparatus with its antenna supplied to an exposure region through tracheostoma, or the laryngoscope is introduced more than once by sticking into the laryngeal tissue at a depth of 5 mm along the perimetre of the involved region. Thermal ablation is performed at temperature 90-100°C. Video-assisted endoscopic surgical removal of the involved tissues is ensured by removing the involved larynx structures, including mucous, submucous and muscle layers within the healthy tissues with using optical magnification of a 4 mm or 5 mm telescope, and a fiberscope introduced through the direct laryngoscope and through tracheostoma. After microsurgical removal of the tissues, high-frequency thermal ablation of the tumour bed at temperature 90-100°C is enabled by sticking the antenna of said device at a depth of 5 mm.

EFFECT: use of the given invention allows preserving skeleton, respiratory, vocal functions of the larynx, reducing probability of postoperative haemorrhages, stenoses, strictures, preventing lymphogenous and haematogenous dissemination of the tumour cells.
