METHOD OF SURGICAL MANAGEMENT OF TROPHIC CORNEAL ULCER Russian patent published in 2011 - IPC A61F9/07

Abstract RU 2421197 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, more specifically to ophthalmology, namely to methods of surgical management of corneal ulcers. It involves a corneal defect closure with an auto-tissue of a Tenon's capsule pedicle flap. Preliminary, depending on the corneal defect localisation, one of adjacent rectus muscles of eyeball are separated. The conjunctiva is dissected above a region of an eye string, the muscle is separated. The eye string and muscle belly are bluntly divided on two equal portions from a muscle fixation to a sclera 10-12 mm backwards. One portion of the muscle is transected at 10 mm away from an insertion site to the sclera. A distal part of the transected portion of the muscle is anchored with interrupted sutures to the preserved part of the cornea with overlapping of a defect surface. In the same region, the Tenon's capsule pedicle flap which is anchored with interrupted sutures around the whole perimeter of a limb is separated after the corneal epithelium removal. A conjunctival wound is closed with interrupted sutures. It is followed with temporary blepharorrhaphy for 2/3 lengths of eyelids.

EFFECT: method enables creating a combined plastic auto-tissue material that eliminates the immunological incompatibility and rejection of a transplant, and also provides the use of revasculisation possibilities of a muscular tissue and Tenon's capsule.
