SUBSTANCE: periosteal-cortical vascularised autograft of the second instep bone is created on an artery of the dorsum of foot with contaminant veins, including a dyaphyseal and metaphyseal plate. A vascular pedicle is separated up to an extensor retinaculum. The extensor retinaculum is dissected. A proximal end of the autograft is removed from the periosteum by stripping and turning off together with feed vessels. At the depth of a bed formed in the shinbone, the bed is formed in an ankle block; between the neck and ankle block, a niche is formed in size of the proximal end of the autograft; the niche is high enough to bring in one of the ends of the periosteal-cortical autograft tightly. The end of the periosteal-cortical autograft separated from the periosteum is brought in the formed niche in the ankle. The vascular pedicle of the autograft is looped to place down. The graft is pressed to the bed surface in the shinbone and fastened with elastic tension. The strip periosteum is placed down the ankle surface and fastened.

EFFECT: method enables reducing the length of fusion.
