METHOD OF DIAGNOSING ATRIOVENTRICULAR NODAL ECTOPIC TACHICARDIA IN CHILDREN AFTER SURGICAL TREATMENT OF VALVULAR DEFECTS BY DETECTION OF ATRIOVENTRICULAR DISSOCIATION ON ECG WITH APPLICATION OF MEDICATIONS (ITS VERSIONS) Russian patent published in 2012 - IPC A61B5/00

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to children's cardiology. Detection of tachycardia with moderate hemodynamics irregularities. In order to detect AV-association traditional methods are used. ECG is registered in 12 standard leads with standard and double enhancement of calibration signal. Location of regular P waves which are seen in different parts of superficial standard electric cardiogram is identified: whether they get on T waves, QRS complexes or isoline. If P waves failed to be visualised, no convincing data for AV dissociation were obtained. Test with ATP is performed with obligatory synchronous registration of ECG in II standard lead during ATP introduction. Doses of ATP for identification of AV-dissociation correspond to conventional doses. Tachicardia with critical hemodynamics irregularity is detected in the following way. AV-dissociation is identified on ECG with application of medications. Amiodarone is introduced intravenously in saturation dose 5 mg/kg in 5% glucose solution. Long-lasting i/v infusion of amiodarone is performed in supporting dose. In case of hypotension aggravation saturation with amiodarone is carried out 2-3 hours earlier. In the process of therapy, with diagnostic purpose, in order to identify AV-dissociation ECG is controlled from monitor.

EFFECT: method makes it possible to diagnose ectopic tachycardia promptly, reduce lethal outcome caused by tachycardia.
