FIELD: medicine.
SUBSTANCE: invention refers to surgery, and may be used for surgical approach to anterior mediastinum organs. At 1.0 cm from sternal borders at the level of I and V intercostal spaces, investing tissues, periosteum and sternum are incised by counter sutures at angles 160-170° opened to each other. The investing tissues are incised along a left border of mesosternum. The II-V rib ends are separated to form a hinged flap in right sternocostal joints to be opened with driving the borders apart with a dilator of brace length equal to the length of the separated sternal fragment. It is followed by a planned operative procedure on the exposed mediastinum organs: thymus removal, aortic valve replacement, coronary artery bypass. The flap is placed back and sutured together with the borders.
EFFECT: method enables reducing intraoperative injuries, reducing a risk of sternal osteomyelitis.
