FIELD: medicine.
SUBSTANCE: group of inventions relates to medicine. Method of treating urine incontinence in male patient lies in: (1) provision of implanted device, containing main part, which has apex and base, at least, first and second elongated elements and at least third and fourth elongated elements, (ii) performance of vertical perineal cut along the median line with lateral dissection for exposure of periurethral adipose tissue above. Bulbar part of patient's urethra and bilaterally downward to branches of pubic bone without involving periurethral tissue, (iii) provision of first introducer, which has introducer tip, (iv) placement of first introducer around first branch of pubic bone, starting from vertical perineal cut from outside inside, medially and through obturator membrane, corresponding to first branch of pubic bone, (v) direction of first introducer tip through obturator hole, corresponding to first branch of pubic bone, (vi) connection of one of said at least first and second oblong elongated elements to tip of first introducer with following pulling said first introducer back, changing its route from outside inside for the reverse one until one of at least first and second oblong elongated elements is free and can be captured, (vii) placement of first introducer around second branch of pubic bone, starting from vertical perineal cut from outside inside, medially, and through obturator membrane, corresponding to second branch of pubic bone, (viii) direction of tip of first introducer through obturator hole, corresponding to second branch of pubic bone, (ix) connection of the other of said at least first and second oblong elongated elements to tip of first introducer and pulling first introducer back, changing route of first introducer from outside inside into reverse one until the other of first and second oblong elongated elements is free and can be captured, (x) pulling of said at least first and second oblong elongated elements in such a way that base of the main part is located somewhat lower than the level of bulbar part of urethra and above patient's pubic symphysis, (xi) sewing at least first and second oblong elongated elements, which pass through each obturator hole, to each other, (xii) performance of a pair of opposite cuts of abdominal wall above pubic symphysis laterally with respect to median line, (xiii) provision of second introducer, which has curved body, continuing from manipulatuion end to introducer tip, (xiv) passing second introducer in front of pubis from above, its introduction into first of the pair of cuts of abdominal wall and bringing it out from perineal cut laterally to periurethral tissue, (xv) connection of one of at least third and fourth oblong elongated elements to second introducer and pulling first introducer back, changing its route for reverse one, and from first cut of abdominal wall, (xvi) passing second introducer in front of pubis from above, its introduction into second cut of abdominal wall and bringing it out from perineal cut laterally to periurethral tissue, (xvii) connection of the other of at least third and fourth oblong elongated elements to second introducer and pulling second introducer back, changing its route for reverse one, and from second cut, (xviii) pulling at least third and fourth oblong elongated elements in such a way that main part of implanted device is located above pubic symphysis, and (ixx) fixation of said at least third and fourth oblong elongated elements inside patient's body. Method of urine incontinence treatment lies in the following: (i) implanted device, which contains main part, having apex and base, at least first and second oblong elongated elements and at least third and fourth oblong elongated elements, is provided, (ii) at least first and second oblong elongated elements are located in such a way that base of main part is located proximally to bulbar part of urethra and above patient's pubic symphysis, (iii) at least first and second oblong elongated elements are fixed by means of obturator hole, with at least first and second oblong elongated elements providing place, on which in fact part of main part is fixed in spatial ratio with obturator hole and patient's entire pelvic area, (iv) at least third and fourth oblong elongated elements are placed in such a way that main part of implanted device is above pubic symphysis, and (v) at least third and fourth oblong elongated elements are fixed inside patient's body in such a way that at least third and fourth oblong elongated elements ensure required tension of implanted device with respect to bulbar part of urethra, which it performs impact on.
EFFECT: inventions ensure easier installation by surgical way with optimal degree of urethra support for effective treatment of urine incontinence with possibility of correcting tension of implanted device after operation by minimally invasive and effective method.
