FIELD: medicine.
SUBSTANCE: invention relates to field of medicine, namely to pediatrics, traumatology and orthopedics and can be used for determination of treatment for children of age to 2 months with detected congenital hip dislocation. X-ray examinations are carried out with determination of indices of Hilgenreiner scheme: acetabular angle, distance d and height h, additionally determined is ratio of acetabular roof width to width of femur proximal end - coefficient K and length of perpendicular L, which is dropped from extreme point of bone edge of acetabulum to the line, connecting anterior superior iliac spine with Y-shaped cartilage. First X-ray examination is carried out after clinical identification of signs of congenital hip dislocation, and if on X-ray image during first examination: coefficient K=0.7-0.8, length of perpendicular L=0-1 mm, acetabular angle equals 37-42°, h=7-8 mm, d=16-19 mm, treatment is started by application of splint-spacer. After 1.5 months X-ray examination is repeated and if during second examination coefficient K increases to 1.2, perpendicular L increases to 3-4 mm, acetabular angle equals 34-36°, h=9-10 mm, d=13-14 mm, treatment by means of splint-spacer is continued. In case, if on X-ray image during first examination coefficient K is lower than 0.7, L=0-1 mm, acetabular angle >42°, h equals 3-5 mm, d equals 25-27 mm, treatment is started with application of complex splint-spacer plus gypsum bandages. If during second examination observed are: increase of K to 1.2, perpendicular L to 3-4 mm, decrease of acetabular angle to 34-36°, increase of h to 9-10 mm, decrease of d to 13-14 mm, further treatment is carried out with application of splint-spacer. If at the moment of second examination parameters do not change or change insignificantly: acetabular angle decreases to 38-40°, h increases to 4-6 mm, d decreases to 22-24 mm, complex treatment with application of splint-spacer and gypsum bandages is continued.
EFFECT: method makes it possible to ensure the most accurate prediction of tactics of treating congenital hip dislocation, identified in children at age to 2 months, due to determination of specific X-ray parameters, which in combination with already known parameters give accurate X-ray picture.
