FIELD: medicine.
SUBSTANCE: transosseous apparatus is mounted. A femoral head is gradually moved into a lower part of the cotyloid cavity. The hip joint is fixed in positions of abduction, flexion and internal rotation by means of pins with the optimum orientation of the head in relation to the cotyloid cavity. Through an incision above the anteroinferior spine, an osteotomy is performed in parallel to an edge of the cotyloid cavity anteroposteriorly, superoinferiorly, from the outside inwards of the ilium to the level respective to the centre of the femoral head. An inner edge of the ilium is intact. A bone fragment is cracked with keeping an articular cartilage intact. It is fixed by means of the pins to a hip support. An anteroexternal rotation of the head is combined with a gradual transposition of the cavity anteriorly, outwardly and downwardly to give the shape and volume of the cotyloid cavity respective to the femoral head. The bone fragments are fixed to produce a repaired bone.
EFFECT: higher joint stability, formation of the spherical cotyloid cavity appropriately sized after the femoral head, improved congruence of the articular surfaces by a rotation of the femoral head combined with an inclination of the osteotomised fragment, reduced intraoperative injuries by the use of a small incision and a possibility of early postoperative mobilisation of the patient by the use of the transosseous apparatus.
