FIELD: medicine.
SUBSTANCE: tongue is fixed and taken out of the oral cavity. The tissues are dissected with the use of a harmonic scalpel. The tongue tissues are dissected in a sagittal plane starting from the tip exactly along the mid-line. A mucous membrane and muscles are laterally dissected on the border of the tongue and the mouth floor with a posterior border min. 2 cm from a posterior edge of a clinically detected tumour. A half of the tongue to be removed is cut across in the transversal direction, and the tongue is horizontally dissected away from the mouth floor. In the distal portions of the tongue, a lingual artery and vein are coagulated with the use of ultrasonic scissors. The mucous membrane of the pharyngeal side surface is sutured onto itself by 2-3 interrupted sutures. The mucous membrane and the muscular tissue of the dorsum stump are sutured from the middle of the sagittal incision of the dorsum stump to the mucous membrane and the muscles of a lateral angle of the root of the tongue by 1 interrupted suture with displacing the dorsum stump 1 to the root by 90°, with the tissues cone-shaped within the root of the tongue. The edges of the mucous membrane of a lower surface of the tongue and the muscular tissue are sutured with the mucous membrane of an alveololingual groove up to an anterior one-third of the dorsum stump end or short of 3.0-3.5 cm from the dorsum stump end. The dorsum stump end is rotated towards the sagittal incision of the dorsum stump by 120°, and the mucous membrane and the muscular tissue of the dorsum stump are sutured by 2-3 interrupted sutures with shaping the tip of the tongue as a parabola. An inferior surface of the mucous membrane of the tongue and the mucous membrane of the mouth floor are sutured by 3-4 interrupted sutures.
EFFECT: method provides ablastic properties, homeostasis, minimal injuries; it accelerates the tissue regeneration processes, provides the optimal morphology and functionality of the resected tongue tissues.
