FIELD: medicine.

SUBSTANCE: group of inventions refers to medicine. A system for eardrum incision and delivery of a tube for equalisation of incision pressure comprises a body, an elongated rod unit and a cam assembly. The body comprises a handle. The elongated rod unit is connected to the body. The rod unit comprises an outer rod, a cutting element, a pusher, a tube for equalisation of pressure, a protective device and a dilator. The outer rod has a blunt distal tip. The cutting element linearly moves inside the outer rod. The pusher is movable above the cutting element inside an inner rod. The tube for equalisation of pressure is movable about the cutting element at the distal end of the pusher. The protective device is movable above the pusher and the tube for equalisation of pressure. The dilator is movable above the protective device. The cam assembly comprises a cam shaft. The cam shaft is rotatably integrated into the body. The cam shaft comprises a first cam profile, a second cam profile, a third cam profile and a fourth cam profile. The cam assembly also comprises a first cam follower, a second cam follower, a third cam follower, a fourth cam follower, a spring and a trigger. The first cam follower is movably connected to the first cam profile and attached to the cutting element. The second cam follower is movably connected to the second cam profile and attached to the pusher. The third cam follower is movably connected to the third cam profile and attached to the protective device. The fourth cam follower is movably connected to the fourth cam profile and attached to the dilator. The spring is pressed between the body and the cam shaft and has a pressed position, wherein a torque moment is transferred onto the cam shaft, and a weak position. The trigger is movably connected to the cam shaft, a first position, wherein the shaft is kept pressed, and a second position, wherein the spring can move into the weak position. When the trigger moves into the weak position, the spring is released that makes the cam shaft move and makes the cam followers move the respective portions of the rod unit in the linear direction to incise the eardrum by means of the cutting element, to dilate the incision and to advance the tube for equalisation of pressure from the protective device into the incision by means of the pusher. A method for making an incision and placing the tube for equalisation of eardrum pressure involves contacting the blunt distal end and the eardrum; projecting the cutting element out of the distal end of the rod for making the eardrum incision with the dilator arranged above at least a portion of the cutting element; projecting the protective device arranged above the cutting element inside the dilator out of the distal end of the rod and inserting it into the incision to extend the dilator; the protective device is arranged above the tube for equalisation of pressure; retracting the cutting element into the rod; retracting the protective device into the rod thereby releasing a distal flange of the tube for equalisation of pressure and coming into the extended position; and pushing the tube for equalisation of pressure from the protective device by means of the pusher arranged inside the protective device thereby releasing a proximal flange of the tube for equalisation of pressure and coming into the extended position. After pushed from the protective device, a mid-portion of the tube for equalisation of pressure is arranged in the eardrum incision, while the distal and proximal flanges are arranged from opposite sides of the incision.

EFFECT: inventions enable placing the tympanostomic tube without the use of a number of instruments and the multi-stage procedure.
