A drug delivery device comprises a piston rod, a body of the drug delivery device; drug holders; a drive unit having at least one driving element. The piston rod comprises an elongated element, which has a distal end and a proximal end with the proximal end having at least two groups of guide pins; at least one group has at least two guide pins aligned along one axis in relation to each other and configured to transfer axial and rotation force from the driving element having at least one recess at a pitch P.

EFFECT: using the invention enables reducing a risk of locking or destroying the piston rod.
