FIELD: medicine.
SUBSTANCE: walls of a seminal cyst are excised with preserving a visceral portion of the cyst wall in the form of a pad intimately connected to epididymis head tissue. After the testicle with the cyst-changed epididymis is brought out into the wound, a visceral leaf of funicular process of the testicle and an epididymis capsule stretched over the seminal cyst is notched up to a loose connective tissue layer separating them from an avascular cyst wall. A parietal cyst wall is enucleated by blunt and sharp dissection circumferentially up to a tight attachment point of the seminal cyst wall to the epididymis tissue. The cyst is opened; its wall is excised along the border to the epididymis tissue leaving the latter intact. The visceral portion of the cyst wall left on the epididymis is stretched with an instrument and photo-coagulated in a dosed power mode: light guide 600 mcm, energy 0.8 J, frequency 8 Hz at a distance of 9-10 mm; The mucosal surface is continuously exposed to staggered laser light for 1 s per a laser field accompanied by forming a superficial whitish scab with a non-shrinking pad; that is followed by controlling homeostasis.
EFFECT: method enables increasing radicality of the surgery and reducing a risk of treatment-induced obstruction of a dust of epididymis at the same time that enables avoiding operated testicle fertility loss.
