SUBSTANCE: invention relates to medicine, to operative urology. An incision is made in the visceral layer of the vaginal membrane of the testis. A 4-5 cm long transverse incision is made in the skin and testicular membranes up to the cyst membrane along the anterior surface of the scrotum above the cyst. The cyst shell is separated from the surrounding tissues along the incision area, two fixing clamps are applied to the cyst wall and the cyst is punctured, the contents are removed. A dye is introduced into the cyst cavity, while the volume of the injected dye corresponds to the volume of the cyst; the dye is removed before opening the cyst. Then the fixing clamps are removed, the cyst shell is opened in the transverse direction, while a surgical clamp is applied to the edge of the shell incision, the cyst shell is screwed onto the clamp, separated and pulled out of the surrounding tissues of the spermatic cord, and the testicle shells and scrotal skin are simultaneously sutured.

EFFECT: method makes it possible to increase the radicalness of the operation while minimizing the risk of iatrogenic obstruction of the adnexal duct, reducing trauma and vascular damage, and the possibility of performing the operation under local anesthesia.
