SUBSTANCE: operation performed according to the neck ultrasound finding is preceded by evaluating a diameter and a position of a perforans vessel extending from the sub-mental artery. That is followed by contouring a planned musculocutaneous flap. In the sub-mental artery, a bordering incision is performed along the contour after pre-contouring. The skin area size is determined by the musculocutaneous flap. Blunt and sharp techniques are used to elevate the tissues of the musculocutaneous flap from the surrounding tissues to form a flap comprising the skin, subcutaneous fat, subcutaneous neck muscle, a portion of the anterior belly of the digastric muscle. A subcutaneous tunnel is formed from the sub-mental area to the oral area. The musculocutaneous flap is partially de-epidermised and moved by rotation through the subcutaneous tunnel into the formed implant site within the defect. The flap tissues are distributed within the formed implant site and sutured to the surrounding tissues.

EFFECT: by minimising the donor damage, the method enables increasing the effectiveness of the surgical management, improving the aesthetic and functional effect.
