SUBSTANCE: invention refers to medicine, namely reconstructive and maxillofacial surgery. Receiving bed in the recipient area is formed by cutting the skin and subcutaneous fat in the preauricular region, followed by delamination of the skin flap and placement of the perforans de-epidermized fat flap on the vascular pedicle in the formed receptacle bed and closing it with an exfoliated skin flap. As a perforans de-epidermized fat flap on the vascular pedicle, a vascularized perforans de-epidermized fat submental flap on the vascular pedicle is used, including a distal portion of the facial artery cut off below the submental artery divergence point, and a submental artery including a perforans vessel separated from an anterior abdomen of the digastric muscle. That is combined with incision of skin and subcutaneous fat in the parotid region. Then, a tunnel is formed in the subcutaneous layer of the soft tissues of the middle and lower zones of the face from the preauricular and parotid regions to the upper boundary of the vascularized perforans de-epidermized submental flap followed by rotation and performing a vascularized perforans de-epidermized fat submental flap in the formed tunnel, placing and fixing it in the receiving bed.

EFFECT: method enables recovering volume-contour deformations of the soft tissues of the middle and lower zones of the face without reducing the function of the donor region, preventing the complications associated with the blood supply disorder of the donor flap, and reducing the length of the surgical intervention.
