SUBSTANCE: invention relates to medicine, namely to neurosurgery, maxillofacial surgery, otorhinolaryngology and oncology. The radial artery autograft is taken on the side opposite to the main surgical access. An S-shaped incision of the skin of the flexor surface of the forearm and an incision of the soft tissues of the head in the form of a “question mark” are used. Initially, an “end-to-side” microvascular anastomosis is applied between the distal end of the radial artery and the middle cerebral artery on the side of the tumor lesion with preservation of blood flow through the radial artery in the proximal part. A permanent bypass extra-intracranial shunt with regulated volumetric blood flow is formed by means of a microvascular anastomosis between the proximal end of the autograft of the radial artery and the recipient vessel of one of the branches of the common carotid artery in the “end-to-end” or “end-to-side” type.

EFFECT: method provides effective prevention of ischemic complications in the case of intraoperative damage to the main arteries of the brain, differentiated revascularization of the brain, improving the functional and cosmetic results of surgical treatment of patients with craniobasal tumors involving the main arteries of the brain.
