SUBSTANCE: diameter of the pulmonary trunk, thickness of the anterior wall of the right ventricle, transverse right ventricular dimension at end diastole is measured. The myocardial fibre lengthwise deformation of the lateral segment of the mitral annulus, movement rate of the lateral segment of the tricuspid annulus, lengthwise deformation rate of the myocardial fibres at the level of the lateral segment of the tricuspid annulus, peak transtricuspidal blood flow in the pulse-wave mode of the tissue Doppler unit in the phase of the early compliance of the heart, peak transtricuspidal blood flow in the pulse-wave mode of the tissue Doppler unit in the phase of atrial systole and a relation thereof, peak transmitral blood flow in the pulse-wave mode of standard echocardiography in the phase of the early compliance of the heart, peak transmitral blood flow in the pulse-wave mode of standard echocardiography in the phase of atrial systole and a relation thereof, movement rate of walls of the right ventricular median segment in the pulse-wave mode of the tissue Doppler unit are additionally determined. A discriminator function is calculated by formula D=1.72249 x La - 0.29256 x SI LZhk + 4.922241 x PS PZhd - 0.88857 x TVI PZhk - 0.82031 x SIR PZhk + 8.153026 x Em/Am PZh TDI + 0.929646 x PZhd - 11.7794 x E/A LZh + 0.627372 x TVI PZhs - 88.3848, wherein La is the diameter of thepulmonary trunk, mm; SI LZhk is the myocardial fibre lengthwise deformation of the lateral segment of the mitral annulus, %; PS PZhd is the thickness of the anterior wall of the right ventricle, mm; TVI PZhk is the movement rate of the lateral segment of the tricuspid annulus, cm/s; SIR PZhk is the lengthwise deformation rate of the myocardial fibres at the level of the lateral segment of the tricuspid annulus, c-1; Em is the peak transtricuspidal blood flow in the pulse-wave mode of the tissue Doppler unit in the phase of the early compliance of the heart, cm/s; Am is the peak transtricuspidal blood flow in the pulse-wave mode of the tissue Doppler unit in the phase of atrial systole, cm/s; Em/Am PZh TDI is the relation of the peak transtricuspidal blood flows in the pulse-wave mode of the tissue Doppler unit (PW TDI); PZhd is the transverse right ventricular dimension at end diastole, mm; E is the peak transmitral blood flow in the pulse-wave mode of standard echocardiography in the phase of the early compliance of the heart, cm/s; A is the peak transmitral blood flow in the pulse-wave mode of standard echocardiography in the phase of atrial systole, cm/s; E/A LZh is the relation of the peak transmitral blood flows in the pulse-wave mode of standard echocardiography (PW); TVI PZhs is the movement rate of the walls of the right ventricular median segment in the pulse-wave mode of the tissue Doppler unit, cm/s. If D>0, a severe degree of the dysfunction is diagnosed, whereas D<0 shows a mild degree.
EFFECT: method enables detecting the cardiac chamber dysfunction at the early stage of the disease and preventing severe complications, reducing a risk of chronic cor pulmonale by conducting the complex analysis of the most informative parameters.
