FIELD: medicine.

SUBSTANCE: root canal is treated by the use of a combination of two techniques: Step Back and Crown-Down by means of manual K- and H-files Pro-Endo from 15 to 22 in size according to ISO. The mechanical treatment of the root canal further proceeds with the use of Pro Taper SX, S1, S2, F1, F2 nickel titanium rotary instruments. An apical plug is then created in the apical one-third of the root canal by introducing the Lioplast spongy substance into the root canal and packing it down. That is followed by medicated treatment, drying and filling the root canal with Protaper gutta-percha points and Adseal sealer according to a lateral condensation technique. Thereafter, the root apex is surgically excised that involves making an incision, separating a mucoperiosteal flap within the root apex, performing cortical plate trephination, enucleating an enveloped cyst, excising the root apex together with the apical plug with using a burr. The cavity is washed with 3% hydrogen peroxide and dried. The Lioplast spongy substance is placed retrogradely into a free space of the canal into a place, where the apical plug was before. The bone cavity is loosely filled with the Lioplast spongy substance, and the wound is closed.

EFFECT: method enables creating an apical support, performing the medicated treatment of the root canal, drying the root canal, filling the root canal with keeping the filling material within the apex, and reducing a recurrence rate.
