FIELD: medicine; therapeutic dentistry.
SUBSTANCE: invention is intended for use in the treatment of chronic forms of apical periodontitis of single-rooted and multi-rooted teeth, aggravated by an iatrogenic factor with an extensive focus of bone tissue destruction, aggravated by root resorption. A computed tomography scan of the area of the tooth to be treated is performed. Anesthesia and isolation of the working field are carried out. The filling material is removed, the carious cavity is prepared, the canals are treated mechanically and medicinally with irrigation solutions, they are dried and sealed, and the integrity of the tooth is restored with filling material. In the process of computed tomography of the tooth area, the structural features of its canal-root system and surrounding anatomical areas are revealed, including associated iatrogenic factors, including fragments of instruments, perforations, resorptions, missed canals, and insufficiently sealed canals. Removal of filling material, preparation of a carious cavity and restoration of lost tooth walls are performed under visual control using an operating microscope, identifying the presence of missed canals. If necessary, fragments of instruments and pins are removed and the perforations are closed, the canals are disobturated, and the medicinal treatment of the dental cavity canals is carried out by sequential washing with irrigation solutions of 5.2% sodium hypochlorite, 40% citric acid or 18% etidronic acid. After that, an antiseptic solution is injected into the canals, a rotating instrument is immersed and, with its help, mechanical and parallel medicinal treatment of the canal is carried out. Next, irrigation solutions are introduced and activated in the resulting reservoir of the tooth cavity by creating a hydrowave effect by oscillations of the endoactivator tip. After drying the canals with paper points, they are sealed with thermoplasticized gutta-percha on a carrier, for example, Thermafil, GuttaCore, with epoxy sealer AN plus, monitoring obturation using visiography.
EFFECT: by activating the process of regeneration of affected bone tissue, the method allows to perform effective treatment of chronic forms of apical periodontitis of single-rooted and multi-rooted teeth, aggravated by an iatrogenic factor with extensive foci of bone tissue destruction, aggravated by root resorption.
