FIELD: medicine.

SUBSTANCE: surgery is carried out in two stages: first, fistula is packed, at that, paracentesis is performed from the inside, from the front chamber, the front chamber is filled with viscoelastic, then a Tenon's membrane fragment is cut off, directed to the front chamber and then directed into fistula with a tool under gonioscopic supervision to provide initial fixation of the fragment. Next, the tool is changed to irrigation trabeculotome. The final fragment fixation in the fistula is performed by directing the trabeculotome irrigation jet. Transition to the second stage is performed: trabeculotomy is performed ab interno for 3-4 hours using the irrigation trabeculotome. The diameter of the excised Tenon's membrane fragment exceeds the fistula diameter. Trabeculotomy ab interno can be either intermittent or continuous. Trabeculotomy ab interno is performed to one and the other side of the sealed fistula zone or on one side. Irrigation trabeculotome is a modified 25G caliber injection needle supplying irrigation solution, and the working part is provided with lateral double-edged cutters. The distal end of the needle is made blunt. If choroidal detachment is observed after fistula packing and trabeculotomy, the suprachoroidal space is drained with outward fluid discharge.

EFFECT: method ensures stable normalization of intraocular pressure reduced operative and postoperative complications, achievment of high visual acuity.
