The database of recommendations is interacted. The database of subjects is interacted. Selection of individual recommendations is based on the algorithm for formation of the individual medical plan of the subject. An analysis of the individual medical plan effectiveness is conducted. Alerts include at least one notification of a change in the diagnosis of the subject, notification of the second level of the subject's medical examination, notification of new medical services. Computer-readable storage devices store instructions executed by one or more processing devices to perform operations that implement the method for formation of the a subject's individual medical plan.
