FIELD: medicine.
SUBSTANCE: first and the second rib are completely removed, the third and the fourth rib are crossed at the spine and resected to the antero-axillary line, without crossing intercostal muscle bundles and releasing from the parietal pleura. Without pneumolysis, a new pleural dome is created by a constricting element with a width of 2 to 5 cm and a length of up to 15 cm. One end of the tape is fixed with nodular sutures at the third sternal-rib joint, its second end is fixed with nodular sutures, with tension, to the nearest non-crossed rib, in the region of the costal-vertebral joint, the lateral edge of the constricting element is additionally fixed to the intercostal muscle bundles and pleura.
EFFECT: additional effective controlled collapse of the lung apex, creation of an additional support for formation of a new pulmonary dome.
