FIELD: medicine, thoracic surgery.

SUBSTANCE: the present innovation deals with surgical treatment of tuberculosis of respiratory organs. For this purpose, out of single paravertebral access one should carry out subperiosteal resection of paravertebral sections of the upper four or five ribs followed by extrapleural pneumolysis and desired pulmonary interference. Then, additionally, one should resect anterior sections of ribs II and III, and rib I should be crossed at sternum and vertebral column. One should dissect intercostals muscles from vertebral column up to posterior-auxiliary line being above the upper intact rib and in intercostal area III. Then comes costal descending and fixing to posterior department of underlying intact rib with the help of Dacron ligatures, moreover, rib I should be fixed to cross-sectional process of thoracic vertebra IV. The innovation enables to achieve the highest costal mobilization and develop "new" pleural cupola at lower level after pulmonary resections at great volumes or at caverna's localization in anterior departments of the upper pulmonary lobe. Thus, it is possible to achieve the congruence of the lung and thoracic wall.

EFFECT: higher efficiency of thoracoplasty.
