FIELD: medicine.
SUBSTANCE: neoadjuvant chemotherapy with cisplatin and subsequent radiotherapy in the mode of dose multifractionation is performed with a 4-5 hours interval between fractions followed by an evaluation of treatment effectiveness. At that, for patients who have achieved complete regression, radiotherapy is continued to a radical dose of 60-65 isoG. Patients for whom the treatment effect is assessed as partial regression and process stabilization, are subject to surgical intervention. Neoadjuvant therapy is carried out in the form of 2 courses with an interval of 3-4 weeks, where the course includes administration of docetaxel at a dose of 75 mg/m2 and cisplatin at a dose of 75 mg/m2. Subsequently, radiotherapy is performed in a multifraction mode of a dose of 1.3 Gy, 2 times a day, with an effect on SOD = 40 Gy estimated by the isoelectric effect.
EFFECT: reduced number of complications due to reduced toxicity from chemotherapy courses, larynx preservation, decreased likelihood of distant metastases development.
