SUBSTANCE: transcorneal endocapsular YAG-laser action on the nucleus of the high-density cataract lens. During the action by laser pulses with a wavelength of 1064 nm, with a focus diametre in the air of 10 mcm, with an energy in the pulse of 1.0-1.7 mJ, with a pulse duration of 2-3 ns (10-9 c), with a pulse repetition rate of 2.5 Hz starting from the upper part of the nucleus and gradually moving along the border of anterior part of the posterior cortical layers and the posterior surface of the lens nucleus, a border is formed, separating the nucleus of the lens from the posterior cortical layers. The laser action is carried out from the posterior surface of the nucleus to the center, and then - to the border of the posterior part of the anterior cortical layers of the lens. The distance between the laser pulses is 1.0-1.5 of the diametre of the cavitation bubble formed as a result of the action. 30-40 minutes after the YAG-laser action, ultrasonic phacoemulsification of the cataract with femtolaser tracking is performed, during which the femtolaser fragmentation of the lens nucleus is performed with the following parameters: the wavelength - 1020-1060 nm, the pulse duration: 200-550 fs (10-15 c), the pulse frequency - 0.1-10 MHz, the focal spot diametre - 2 mcm, the energy in one pulse - 25 nJ - 2.5 mJ, the total energy of the femtolaser - 30-33 J.

EFFECT: minimal loss of endothelial corneal cells is achieved by reducing the energy of the femtolaser used at the fragmentation stage of the cataract lens nucleus, as well as reduction of ultrasound energy during cataract phacoemulsification are ensured.
