FIELD: medicine.

SUBSTANCE: for surgical treatment of retinal detachment, cataract phacoemulsification with intraocular lens implantation is carried out. Then trocars are used to set three "ports" 25 or 27G calibre through three transconjunctival punctures in the flat part of the ciliary body, receding 3.5-4 mm from the sulcus. After this, the front cortical layers of the vitreous body are contrasted and the contrasted vitreous body is removed. Contrasting of the vitreous body and its and subsequent removal is performed repeatedly until the vitreous layer located on the posterior capsule of the lens is contrasted. The vitreotome needle is used to remove the posterior capsule of the lens with a diameter of 4-5 mm in the central zone. After complete removal of the retrolental vitreous body, vitrectomy is performed at the periphery behind the posterior capsule of the lens, sclerocompression is performed if necessary. The peripheral VB is also repeatedly contrasted for its visualization and complete removal, at that, VB is removed from its periphery near the eyeball wall, in the region of the VB base and removal is continued throughout the VB base, and the cortical layers are removed from the eyeball periphery and removal is continued in the direction to the central parts of the retina. After the complete removal of the vitreous body, the surface of the retina is again contrasted, especially in the areas of its folds and scars. The preretinal vitreous body is removed, then the vitreal cavity is swabbed with PFOC or a gas-air mixture, depending on the localization of the retinal ruptures. After the adaptation of the retina, endolaser coagulation of the retina is completed, and the operation is completed with tamponade of the vitreous cavity with silicone oil or a gas-air mixture.

EFFECT: method increases the visual function by reducing postoperative complications, such as tractional retinal detachment caused by incomplete removal of the vitreous body, anterior and posterior proliferative retinopathy.
