FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to urology and physiotherapy, and can be used to prevent cicatricial sclerotic complications after surgical treatment on the upper urinary tract. To do this, drain the upper urinary tract by percutaneous puncture nephrostomy, antibiotic prophylaxis. Intramuscularly injected Longidase 3000 ME once every 3 days with a course of 10 injections. Starting from the next day after the installation of nephrostomy drainage, external magnetic laser therapy is performed in the amount of 10 procedures. On the 7th day after the installation of nephrostomy drainage, a course of multichannel electrostimulation (MES) is carried out in the areas of the trapezius and latissimus muscles of the back, direct and oblique abdominal muscles with bipolar pulse currents with a trapezoidal envelope of 1–150 Hz, a parcel and a pause of 2 seconds, and an exposure time of 15–20 minutes. MES is combined with Longidase 3000 ME ultraphonophoresis in the projection of the affected ureter with an intensity of 0.2 W/cm2 in continuous mode is labile. Course of treatment is 10–12 combined procedures, conducted daily. In parallel with the above course of treatment, hyperbaric oxygenation (HBO) procedures are performed in a common pressure chamber with a partial oxygen pressure of 1.25 kgf/cm2 in the mode of isopression. This pressure value is achieved by a stepped compression method at a rate of 0.06–0.08 kgf/(cm2*min), stopping at each compression stage for 3–4 minutes and checking the reaction of the patient to the procedure. In the absence of the phenomena of oxygen intoxication, the duration of the procedure in the isopression regime is 25–35 minutes, during which the decompression is performed for 6–8 minutes. Course of treatment of HBO is 8–10 procedures, conducted daily.
EFFECT: method provides the fastest recovery of patency and peristalsis of the ureter and improvement of the passage of urine, reduction of urodynamic disorders of the kidney, improvement of morphological changes in the ureter's wall and intragroup nerve elements as a result of complex pathogenetic influence, including defibrizing, breaking and depolymerizing effect on connective tissue structures.
