SUBSTANCE: invention relates to medicine, preferably to hepatobiliary surgery, and can be used for parenchyma-preserving right-sided expanded hemihepatectomy. Surgical treatment of portal cholangiocarcinoma is carried out, which includes extended right-sided hemihepatectomy with preservation of the 4a segment of the liver. For this, the right hepatic artery and the right portal vein are ligated. Next, isolate, cross and stitch the right hepatic vein. Intraparenchymally bind and intersect the middle hepatic vein with an indentation of 4–5 cm from the place of its entry with the left hepatic vein into their common venous trunk. Form the border of the division of the liver 5 mm to the right of the right edge of the preserved section of the middle hepatic vein.

EFFECT: method ensures preservation of a larger volume of functioning parenchyma of the liver, a reduction in the traumatic nature of the surgical intervention due to preservation of the 4a segment of the liver, id est more anatomical removal of affected segments of the liver when performing an extended right-sided hemihepatectomy.
