FIELD: medicine, hepatobiliary surgery.

SUBSTANCE: trocars are installed — 5 mm above the navel, the second along the midline at the level of the edge of the liver and two 5 mm trocars in the projection of the bottom of the gallbladder and along the anterior axillary line in the right hypochondrium. Traction is performed for the bottom of the gallbladder up and cranially, the edge of the liver is raised by 80–90°. With the help of Hopkins 26003BA anterolateral optics 30° the hepatoduodenal ligament is visualized from the lateral side. With Click'line 30310RS endoscopic forceps with right-angled jaws, the right branch of the portal vein is precisely isolated along the entire circumference. The vein of the 4th segment is visualized. The right branch of the portal vein is clipped proximal to the level of origin of the vein of the 4th segment, then the vein of the 4th segment is clipped.

EFFECT: method provides increasing the efficiency of preparing patients for extensive resection interventions (hemihepatectomy, extended hemihepatectomy), allows in a short time to achieve a reduction in blood flow along the right branch of the portal vein due to the simultaneous clipping of the right branch of the portal vein and the vein of the 4th segment.
