SUBSTANCE: invention refers to medicine, namely to maxillofacial surgery. Endoscopic access to the styloid process is performed by performing a linear incision of the skin in the hairy part of the occipital region from the projection of the condylar fossa of the occipital bone downward and proximal to the midline of the neck along the hairline. Dissect the subcutaneous fat, the superficial fascia, the superficial lamina of the fascia and the fibers of the trapezius muscle. Lateral portion of the fibers of the ascending part of the trapezius muscle is withdrawn upwards. Isolate, dissect and divert the lesser occipital nerve to the upper corner of the wound. In the upper parts of the scapulaclavicular triangle of the neck produce a dissection over the deep lamina of the fascia. Under the control of a rigid endoscope, a tunnel is formed parallel to the projection line of the edge of the body of the lower jaw. Fascial compartment of the sternocleidomastoid muscle in the region of its posterior leaf is opened, sub-muscular dissection is performed and the muscle is lifted up, the posterior belly of the digastric muscle are visualized and removed. Continue dissection forward and down, dissect the stylopharyngeal fascia and perform osteotomy of the styloid process above the place of its attachment to the lesser horn of the hyoid bone.

EFFECT: method reduces postoperative complications, which is achieved due to the performed access, which allows creating clinically significant diastase between fragments of the styloid process.
