FIELD: medicine.
SUBSTANCE: invention relates to medicine, particularly to reconstructive hand surgery. Through the distal third of the first metacarpal bone in the frontal plane, a bulb-tipped pin is introduced along the ulnar bone. After completion of the metacarpal distraction, the first back interosseous muscle is cut off from the distal fragment of the donor metacarpal bone. Linear cuts of the skin are performed along an apex of the skin fold formed in the interdigitalis pedum. Dorsal and palmar skin-vascular pedicles are formed from the stretched tissues, the distal fragment of the donor metacarpal bone on two pedicles is transferred to the first finger residual limb. Tendon part of the first dorsal interosseous muscle is displaced and fixed transosally to the middle third of the third metacarpal bone. First metacarpal bone is fixed by a pin in the position of maximum abduction transarticularly. Skin defects are substituted in the area of the first interdigitalis pedum and the formed first finger with a free skin graft.
EFFECT: method restores the length of the first finger, prevents ischemia of the first interdigitalis pedum and adduction contracture of the first metacarpal bone.
