SUBSTANCE: group of inventions relates to medicine, specifically to a system and method of clinical support for transferring a patient between care areas using a computer. System comprises a computer-readable storage medium, which contains instructions for performing a method for facilitating patient transfer between care areas using a computer. Method comprises steps of: obtaining current data on a patient currently receiving care in a care area, wherein the current patient data contain data relating to the patient as applied to the current time period. Patient's medical history data are retrieved, the patient's medical history data containing data relating to the patient for one or more time periods prior to the current time period. Scores for the transfer of a specific patient from said current data obtained or the patient's medical history data are calculated, wherein transfer scores of a specific patient are used to form a recommendation for transferring that patient from the current care level to another care level or leaving at the current care level. Weights are calculated for transfer scores of a specific patient, manually or based on the accuracy of previous transfer scores for that patient and the accuracy of previous transfer scores for other patients, and the calculation of the total transfer scores. Weighted combination of said total transfer scores and said transfer scores for the specific patient is calculated to obtain the final transfer scores. System also includes means for obtaining current data about a patient receiving care in the current care area, wherein the current patient data contain data relating to the patient for the current time period. Means for obtaining patient’s medical history data, wherein the patient's medical history data contain data relating to the patient for one or more time periods prior to the current time period. Means for calculating the scores of a particular patient's transfer based on the current data obtained and the patient's medical history data, wherein the transfer scores of the specific patient used to formulate a recommendation for transferring that patient from the current care level to another care level or leaving at the current care level. Means for determining weights for transfer scores of a specific patient manually or based on the accuracy of previous transfer scores for that patient and the accuracy of previous scores for other patients and the calculation of the total transfer scores. And means for calculating a weighted combination of said total transfer scores and said transfer scores for the specific patient to obtain the final transfer scores.
