FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to surgery. Access to the lip of the hernia is performed by dissecting skin and subcutaneous fat above the hernial protrusion with excision of the postoperative cicatrix. Hernia sac is separated from surrounding tissues to cervix without opening. Subcutaneous fat is detached from the aponeurosis at distance of 4–5 cm from the border of the lip of the hernia. Hernia sac is dissected into 2 flaps. One of the flaps is of a smaller size and corresponds to the size of the lip of the hernia. Adhesions between organs of abdominal cavity and hernia sac are separated, and contents of hernia sac are immersed into abdominal cavity. Lip of the hernia are covered with a smaller flap and fixed to the aponeurosis edge with interrupted sutures. Mesh implant is laid on a smaller flap with the condition that it extends beyond the lip of hernia by 2–3 cm, and fixed to the unchanged aponeurosis with interrupted sutures, and thorough haemostasis is provided. Then the mesh implant is covered with a larger-sized flap with an exit of this flap along the entire perimeter of implant for 1–2 cm, which is also fixed with interrupted sutures to the aponeurosis; the wound is closed.

EFFECT: method enables reducing the number of recurrences, reducing hospital stay, reducing the number of local early and late postoperative complications, as well as reducing pain syndrome.
