METHOD OF SPONDYLOSCOPIC APPROACH IN THE AREA OF CRANIOVERTEBRAL JUNCTION AND UPPER CERVICAL SPINE WITH BLOOD-EXERTED AUTOLOGOUS FLAPS Russian patent published in 2019 - IPC A61B17/56

Abstract RU 2687777 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for spinal fusion in the area of craniovertebral junction and upper cervical spine with blood-exerted autologous flaps. Median longitudinal incision of the skin and subcutaneous fat from the prominence of the occipital bone to the spinous processes of the operated vertebrae is performed. Nasal ligament is incised longitudinally. Intrinsic fascia of the neck is separated from the spinous processes on both sides and the scales of the occipital bone and the posterior half-arch C1 of the vertebra are skeletonized. Inferior oblique muscle of the head is extracted from the muscular flap by dissecting fascial spaces, and when the wound edges are diluted, the lower oblique muscle of the head remains attached to the spinous process of the C2 vertebra. Vertebral spinous process is split longitudinally with transition of an osteotomy line to a plate portion of an arch of a C2 vertebra and splitted on a spongy layer. Blood-supported bone flaps are formed, consisting of halves of spinous process, external cortical layers and part of spongy layer of arc of process C2 of vertebra on feeding stem from own vessels of lower oblique muscle of head. Obtained blood-preserved bone flaps are removed to the outside, moved upwards and fixed to pre-decorticated surfaces of the occipital bone and processes of C1 and C2 vertebras.

EFFECT: method provides complete bone adhesion, reduced risk of infectious complications.
