SUBSTANCE: invention can be used in the surgical treatment of adults and children with atlantoaxial instability. The method includes cutting the skin and subcutaneous tissue along the line of the spinous processes, skeletonization of the spinous processes and arches of C1 and C2 vertebrae, opening the capsules of the atlantoaxial joints along their posterior surface, removing the hyaline plates of the articular surfaces of the atlantoaxial joints, repositioning the deformity and distraction of the atlantoaxial joints under conditions of drug relaxation, introduction of tricortical autologous iliac bone grafts into the interarticular spaces, posterior screw fixation of C1-C2 vertebrae, layer-by-layer wound closure. At the same time, during the method, the posterior edges of the upper articular facets of the C2 vertebra and the anterior edges of the lower articular facets of the C1 vertebra are resected until a parallel orientation of the articular spaces of the atlantoaxial joints relative to the axial plane of the body is achieved. Tricortical autobone grafts formed from the iliac crest are inserted into the interarticular fissures of the atlantoaxial joints.

EFFECT: method provides increased efficiency of treatment by providing fixation of the atlantoaxial joints and increasing the frequency of bone block formation in the spondylosynthesis zone.
