FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgery, and can be used for manual assisted laparoscopic splenectomy with closed abdominal injury. Carboxiperitoneum 12–14 mm Hg is applied. Video laparoscopy is performed to assess the nature of spleen damage and intensity of haemorrhage. Mini-access is performed. Access device comprising a wound retractor and a cover with a diaphragm for inserting a surgeon's arm is inserted. Together with the surgeon's hand, a polyethylene tube with diameter of not less than 8 mm and connected to the electric pump is introduced into the abdominal cavity. Left sub-diaphragmatic space is dried with using a tube controlled by a surgeon's hand and an endoscopic instrument. After opening the omentum bag and exposing the pancreas to the upper edge of the pancreas with the help of the thumb and first fingers of the left hand and the endoscopic instrument in the right hand, the splenic artery is first picked, clipped and transected, followed by ligation of the short gastric vessels. Spleen is extracted in the container through an access device. Access device is removed.

EFFECT: method provides good visualization and low traumatism, simplified splenectomy.
