FIELD: medicine.

SUBSTANCE: during vediolaparoscopic gastrectomy without concurrent splenectomy, with extended lymphadenectomy during cancer of the proximal section and middle third of the stomach, during vediolaparoscopic partial distal gastrectomy with extended lymphadenectomy the lesser omentum is mobilised. Left and right gastric arteries and veins are tied up. The gastric-pancreatic ligament is transected. The laparoscope and working instruments are let in through the opening formed on the lesser omentum. The laparoscope is moved along the top edge of the pancreas and splenic vessels to hilus of the spleen.

EFFECT: reduced injury of operation, risk of instrumental damage to the spleen, tail of the pancreas, upper pole of the spleen, wider range and sharpness of imaging during surgical manipulations in the region of splenic vessels.
