FIELD: medicine.
SUBSTANCE: invention refers to surgery and can be used for correction of varus strain of knee joint and retardation of gonarthrosis progression by means of mini-invasive operating technique of high tibial osteotomy. Device for fixing bone fragments with high tibial corrective osteotomy contains an insert, a plate, bone screws with angular stability and a closing screw. Insert is made wedge-like and has a body formed by two converging support planes for contact with bone and side surface. Lateral surface has areas passing one into the other: the first portion is curved in the shape of a shin-like bone surface in the osteotomy and lies at the point of greatest expansion between the support planes, second portion abuts on one side of first portion and is flat, third portion abuts on other side of first portion and is serially made convex, then concave and again convex or flat or wavy with transition to fourth section made flat with inclination at acute angle to second section with rounding at connection point. Support surfaces for contact with bone are toothed. Insert has a threaded hole for the locking screw perpendicular to the plane of the second section. Insert is made porous, but in the area of the threaded hole - solid. Plate is shaped and has a length: a straight portion passing from one end into an enlarged area located on one side, oriented to the front of bone, and on the other end - in a stepped zone located on the other side, oriented behind the bone, wherein the steps are located at obtuse angles to the straight part and to each other. Straight part of the plate on the side adjoining the bone is made in the middle of the flat for contact with the second flat section of the insert, and on the expanded and stepped zones it is made with surfaces similar to bone surfaces, to which the above zones adjoin. In the middle of the flat part of the plate there is a hole with a chamfer for the closing screw, and on the expanded and stepped zones, on each, there are three threaded holes for threaded heads of bone screws, with location of axes of these holes in apexes of conditional triangles.
EFFECT: technical result is preventing injury (compression) tendons "crow's feet" after plate implantation, loss of correction and high strength of fixation due to screws with angular stability.
