METHOD OF TREATING DOUBLE-SIDED CROSS BITE IN PATIENTS WITH UPPER JAW CONSTRICTION Russian patent published in 2019 - IPC A61C7/00 A61C8/00

Abstract RU 2699532 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine and can be used for treating double-sided cross bite in patients with upper jaw constriction. Local anesthesia is performed. Incision of a mucous membrane of an anterior surface of the upper jaw is performed in a gingival edge of the upper jaw from tooth 1.7 to 2.7. Anterior surface of the upper jaw is skeletonized within the boundaries from the apex of the alveolar process to a level slightly above the bottom of the nasal cavity and from the tuberosity of the upper jaw on one side to the tuberosus of the upper jaw on the other side through the respective bilge alveolar ridges. Nasal cavity mucosa is detached in anterior region within pear-shaped opening. Osteotomy of the anterior surface of the upper jaw is performed in parallel to the thickness of the cortical plate on both sides of the piriform aperture through the bilal-alveolar ridges to the maxillo tuberosises inclusive in the middle of the distance from the apices of the dental roots to the projection of the topographic position of the maxillary artery. From the oral cavity side, the solid palate is skeletonized from the second molar to the central incisor towards the median palatine suture until it intersects with the middle suture of the hard palate. On the side of the oral cavity from the alveolar process, incision is made along the median line on the thickness of the cortical plate from the lower edge of the pear-shaped opening, crossing it to the apex of the alveolar process inclusive. From the vestibular side of the alveolar process, a saw cut is made along the median line on the thickness of the cortical plate from the lower edge of the pear-shaped opening, crossing it to the apex of the alveolar process inclusive. Wide chisel is used to mobilize maxilla bone fragments according to Le Fort III and relative to pterygoidei processes of the sphenoid bone in sagittal and transversal planes. Wound is closed with interrupted sutures. Apparatus with Hyrex screw is installed for forced opening of median palatine suture, which is fixed to teeth 1.4, 1.5, 2.4, 2.5, 1.6, 2.6. Median palatine suture is opened by activating the screw according to a pattern: on the first day, the opening of screw is 0.9 mm, and then, for 12–16 days, the screw is increased by 0.45 mm at a time. After achieving the required opening of the palatine suture, the position of the screw is fixed. Denture orthodontic correction device is installed on the upper jaw teeth. Apparatus with Hyrex screw is fixed fixed to teeth of upper jaw to achieve stable expansion of upper jaw.

EFFECT: method solves the problem of simplifying the method with a simultaneous increase in the therapeutic effect, also enables reducing the probability of postoperative complications, reducing the length of treatment, providing universality, enlarging the range of products for complex rehabilitation of patients with combined dento-jaw anomalies, such as a cross bite with narrowing of the upper dentition.
